Welcome to the Third Annual PCF National Meeting

In the opening plenary, you will hear from CMS leadership as we reflect on the evolution of value-based primary care and share our optimism for the future thanks to the trailblazing work of your dedicated teams. You will also hear about the state of the PCF Model and how your work on the ground translates to improved outcomes and a transformative care experience for patients.

    Second PCF Annual Report Review Session
    Evaluation of the Primary Care First Model

    All CMS Innovation Center Models are rigorously evaluated to look at provider and patient experiences with a model, transformation of the health care system, and a model’s impact on spending and quality of care. In this session, we will cover findings from the first two years of the PCF Model (January 2021 – December 2022) in the Second Annual Report. Learn about how practices use model funds to initiate care delivery changes, aim to reduce unnecessary hospitalizations, and how practices’ prior experiences and affiliations aided this process.

            From Collection to Action: A Data-Driven Approach to Addressing Health Inequities

            In this session, learn how a system practice in Massachusetts uses race, ethnicity, and language (REaL) data to identify potential health disparities. Hear firsthand how they’re identifying potential inequities among their patient populations by stratifying quality measures (i.e., Blood Pressure, Diabetes A1c) based on this demographic data.
